Diana has been an Occupational Therapist since 1990 and has worked in a variety of setting including Baptist hospital and JOI.   She is also a Certified Hand Therapist since 2000. She continues to be an asset to this rehabilitation office with her knowledge of the functioning of the hand. Diana is also fluent in Spanish.

Chad has been a licensed physical therapist since 1999.  He has worked in a variety of outpatient settings. He is an original member of the JOI rehab team. Since 2009 he has been the Site Coordinator for JOI’s University office and has now transitioned to being the Site Coordinator at JOI’s Arlington office. For the past 7 years, he has been the team physical therapist for Jacksonville University’s athletic program.  He continues to stay up-to-date with the latest in orthopedic care while rooting for the Gators and Jags.

Prior to joining JOI, Katie managed an outpatient clinic working with people with various orthopedic conditions and helping with local high school athletics. She has worked in the outpatient setting since graduating from the University of St. Augustine, where she focused on manual therapeutic techniques and sports medicine. Katie found her passion for physical therapy while playing soccer in high school, club, and collegiately, and still enjoys running, yoga, and Jaguars football. She is interested in pursuing certifications in manual therapy and orthopedic specialist.

Anita is originally from Orlando, Florida and graduated from the University of Central Florida with a B.S. in Health Science. She then graduated with honors from the University of St. Augustine in 2017 with a Master of Occupational Therapy degree. She is currently working toward obtaining her Certification in Hand Therapy.

At the University of Florida he had the opportunity to assist with treatment of Division I collegiate athletes in a variety of sports and professional athletes in the National Football League (NFL). He has been a physical therapist for the touring companies of the Broadway shows Wicked and Shrek the Musical. He also served as a physical therapist for the Atlanta Falcons (NFL) and Georgia Force (AFL) cheerleading squads. Steve has treated Ironman and ITU World Championship triathletes, and has been a member of high school sports medicine teams. Steve joined JOI in 2014 and brings his interest in manual therapy and sports medicine to our clinic.

Joan worked at Baptist Medical Center in the Health/Wellness/Cardiac Rehab department for nine years. After receiving a second PT degree, she enjoys working in inpatient/outpatient hospital settings, home health, skilled nursing facilities, and outpatient Sports Medicine physical therapy. She is trained in the Graston Technique, Electric Therapy Point System (ETPS), McConnell Taping, Body Balance for Performance Golf Program, and personal fitness programs. Formally with the Aspen Colorado Hospital Physical Therapy Department, Joan joined JOI Beaches Rehab in 2005, and is currently the Senior Physical Therapist.

Brendan has been a part of athletic events ranging from youth baseball, middle and high school football, all the way up to seniors men’s lacrosse. He spends his free time either fishing, or with his family and 2 dogs.

Shane Nall is a native of Jacksonville Beach and a local around the beaches community in his 26 years of living. As a graduate of The Florida School of Advanced Bodywork in Jacksonville, formally known as the Southeastern Schools of Neuromuscular Massage, Shane is well versed in Structural Integration Bodywork and Neuromuscular Massage Modalities. His focus is identifying the causative factors of muscular pain and how posture can create and form muscular imbalances that cause structural pain. With a deep understanding in the anatomy, physiology, and kinesiology of the body, Shane uses multiple Medical Massage modalities to treat the pain people experience throughout daily life such as; Neuromuscular Therapy, Trigger Point Therapy, Myofascial Release Technique, and Sports Massage.

Massage Therapy and Bodywork is Shane’s passion, he is a member of the American Massage Therapy Association (AMTA) and currently working towards growing as a LMT, with multiple courses and certifications lined up in the future. Shane works at both our Beaches and South Rehabilitation facilities.

In his free time, Shane enjoys traveling to different surf and winter snowboard destinations. He also scuba dives and has a passion for any fun outdoor adventure.

Lisa received her Bachelor of Science degree in Athletic Training from Duquesne University (Pittsburgh, PA) in 2007. She continued on to earn a Masters degree in Exercise Science while working as a Graduate Intern at Edinboro University of Pennsylvania, where she was the Athletic Trainer for the Men’s Football, Wrestling, and Women’s Softball teams. After moving to Florida, Lisa worked as the Head Athletic Trainer at Flagler Palm Coast High School in Palm Coast, FL. Lisa joined JOI in July of 2012, and is currently the Head Athletic Trainer at Bishop J. Snyder High School, providing medical coverage for all Cardinal student-athletes.

She is actively involved in the JOI Sports Medicine Program where she serves as assistant sports medicine director and helps with sports medicine coverage at the high school and collegiate level. Sonya also works closely with the Jacksonville Juniors Volleyball Association (JJVA), helping to provide sports medicine coverage for all of their home tournaments.

Darby graduated from the University of North Florida in the Spring of 2017. She worked with Division I football at Jacksonville University and Division I softball at UNF as well as a few local high schools. She currently works at Fernandina Beach High School as their Head Athletic Trainer. Darby became interested in athletic training growing up as a competitive gymnast and then competing in collegiate cheerleading before entering the Athletic Training Program.

Adam is originally from the Chicagoland Area and moved to Jacksonville shortly after graduate school.  While in PT school Adam fulfilled multiple clinical rotations all over the country, with a special focus on orthopedics and sports medicine. During his final year at the University of Dayton, Adam was able to complete an Independent Study at DePaul University in Chicago working with Division I college athletes. Adam started working at JOI in June 2018 and currently splits his time between the San Marco and Nassau clinics. In his off time Adam enjoys exercising, playing soccer and spending time outside with friends and family.

Laura graduated with honors from University of Illinois at Chicago in 1988. She has spent her career focusing on orthopedic upper extremity rehabilitation. Laura became a Certified Hand Therapist in 1993. Since that time she has taken many opportunities to further her education through continuing education, membership in professional organizations such as American Society of Hand Therapist, hand study groups and further certifications such as; Graston soft tissue technique, kinesio taping, LSVT BIG. Laura takes a hands on approach to therapy and her goal is to achieve the highest level of function for her clients. Throughout her career Laura has always worked closely with the hand surgeons she works with to provide the best care for her clients through good communication regarding diagnosis and treatment options.

Amelia is a recent graduate from Florida State College of Jacksonville. During her clinical experience, Amelia focused on sports and Orthopaedic injuries. Not only did she work with younger athletes, but also had the opportunity to work with older athletes who just "wanted to stay in the game." Prior to becoming a Physical Therapist Assistant, Amelia attained a Bachelors of Arts in Dance Studies from the University of South Florida and is classically trained in ballet and modern. Upon graduation, Amelia presented a dissertation explaining the benefit of having biannual physical therapy screenings on the students of the dance department. Amelia is looking towards teaching dance again in her spare time and is excited to continue on her journey with JOI.

Michele has been a long time Florida resident, before moving to Jacksonville 1.5 years ago, she resided in Port Orange on central Florida’s coast.  She has worked in the medical field for nearly 20 years in several different settings.  Before joining the JOI team, Michele worked in geriatric home care NICU as a CNA, assisted living, dementia & Alzhiemer’s care, and mental health.  In her spare time, Michele enjoys spending time cooking, gardening, crafting, fishing, watching her beloved Patriots and learning more about several interests such as small-scale farming and self-sustainable living.

Alex is a Florida native who grew up playing baseball.  He went on to play baseball for Chipola College while earning his A.S. before transferring to the University of Central Florida to focus on his bachelor’s degree.  Alex then decided to attend FSCJ to become a physical therapist assistant.  Since becoming licensed, Alex has clinical experience in outpatient orthopedics, skilled nursing rehab, and balance/vestibular rehabilitation. Alex has been a member of the JOI rehab team since 2018.

Justin is a graduate from the University of St. Augustine for Health Sciences with a specific focus in manual therapy. During his clinical experiences he treated a wide range of sports and orthopaedic injuries and had the opportunity to work directly with various collegiate and professional athletes. Prior to becoming a Physical Therapist, Justin graduated from the University of Cincinnati, with a Bachelors of Business Administration, and worked as a Sales/Marketing Manager for an International Manufacturing Supplier. Justin has a passion for therapeutic exercise program design/implementation, functional movement analysis, and sports injury prevention. He is currently working towards obtaining his Manual Therapy Certification from the University of St. Augustine for Health Sciences and plans to further his knowledge of functional movement analysis/sports injury prevention by pursuing a Functional Movement Systems Certification. Justin’s hobbies include cabrewing, whitewater rafting, kayaking, fantasy football, finger painting, zumba, competitive eating, and powerlifting.

Ehren Allen performed his undergraduate training in Music at the Florida State University. He went on to complete his professional training in Physical Therapy at Florida A&M University. He practiced in many specialty areas since 2000, including neurology, pediatrics, and private practice before deciding to specialize in orthopedics and sports medicine. Daniel is a New Jersey native who graduated from Rider University (2013) with a Bachelor’s in Behavioral Neuroscience, where he  was a member of the Swimming and Diving team. He went on to pursue his Doctor of Physical Therapy degree from The University of Florida and graduated in 2017. Daniel fulfilled multiple internships while at the University of Florida, including an outpatient sports rotation at the Texas Metroplex Institute where he had the opportunity to work with athletes of all age ranges and gained a strong interest for outpatient orthopedics. He is excited to be part of the JOI team.

Luke is a graduate from University of North Florida with a major in Athletic Training. His clinical experience includes Division-I athletics with the UNF softball and soccer team. He was also part of the Sports Medicine staff for Jacksonville University Football. He is currently the Athletic Trainer for Fernandina Beach High School Athletics, providing sports medicine coverage for practices and games. Luke focuses his treatment based on a combination of manual therapy and therapeutic exercises to build strength, stability and improve ROM. Luke enjoys playing beach volleyball, kayaking, swimming, and playing guitar.  Luke is now the ATC for the Jacksonville Sharks.

In the Fall of 2017, she continued her education by working on her Master’s degree in Advanced Athletic Training at the University of South Florida. Samantha worked for two years as an intern at Warner University in central Florida where she was primarily responsible for men’s soccer and baseball. Samantha joined JOI in July of 2018 and is serving as the Head Athletic Trainer for Atlantic Coast High School.

Jessica  is originally from New Jersey and moved to Florida for graduate school. She decided to go back to school after being exposed to occupational therapy, and falling in love with the career. She thoroughly enjoyed working in the outpatient setting and decided to pursue her career in this setting. She is excited to begin her career at JOI.

Bridget has been a practicing physical therapist assistant here at JOI since November 2017. She was born and raised here in Jacksonville where she attended Duncan U. Fletcher High School.  She then went onto West Virginia University where she walked onto the Varsity Volleyball team and competed in the Big East as a Libero/ Defensive Specialist.  She then went on to transfer to the University of Central Florida where she completed her Bachelors in Science in Communication Sciences and Disorders.  She found her love for the field of Physical Therapy during her last semester at UCF while working with patients in the Aphasia House where she was given the opportunity to be apart of the rehabilitation of patients that were affected by TBI, stroke, and other neurological deficits. She then attended the PTA program at FSCJ and graduated in August of 2017.  In her free time she enjoys playing beach volleyball, riding her beach cruiser, and playing with her two dachshunds.

During his clinical internships he treated both traumatic and non-traumatic brain injury patients at Brooks Rehabilitation Hospital and placed special emphasis on upper extremity orthopedics at the Malcom Randall VA Medical Center. Prior to becoming an Occupational Therapist, Chang graduated from the University of North Florida with a B.S. in Health and Exercise Science and is a Certified Exercise Physiologist through the American College of Sports Medicine. Michelle gained valuable experience at UF by working with the volleyball and football teams. She later interned at the University of South Florida and worked with the football team, which was invited to the Meineke Car Care Bowl, the first bowl game in USF history. While at USF, she worked mainly with the Women’s volleyball team, and helped to cover the NCAA Women’s Final Four in 2008 and the NCAA Men’s First and Second Rounds hosted in Tampa. She also helped host the Big East Tennis and Baseball Tournaments in 2008. She came to JOI in 2008, and is presently the athletic trainer for events and practices at Florida State College at Jacksonville. Michelle also serves as Sports Medicine Director.

Ben graduated from Florida State University with a B.S. in Psychology (2007) and from the University of St. Augustine program with a Doctor of Physical Therapy (DPT) degree (2014). He has worked primarily in outpatient orthopedics treating surgical and non-surgical conditions from the neck to the foot and vestibular dysfunctions. His doctoral studies developed a strong background in manual and orthopedic physical therapy and allowed him the opportunity to complete all courses for his Manual therapy Certification (MTC) from the University of St. Augustine. In his off time, he enjoys working out, surfing and spending time with his friends and family.

Kerri is a Jacksonville native who graduated from Bishop Kenny High School in 2010. She received her Doctorate of Physical Therapy from the University of Florida in April 2017 and began working for JOI in August 2017. While in PT school Kerri completed an internship in Arlington, TX working with a large sports medicine facility gaining experience in post-operative treatment and high level athletic training. She enjoys treating an array of patient populations and has a special interest in post-concussion rehabilitation. Kerri is active in the American Physical Therapy Association, attending the annual conferences in order to stay current with evidence-based practices and techniques.

Thomas has been a certified athletic trainer since September of 2010. He has experience  working with a multitude of sports with a vast range of age groups. Originally a Massachusetts native he has traveled to London to work with cheerleaders and dancers. He has worked with professional athletes doing a summer internship with the St. Louis Rams, working with the Bridgeport Bluefish (Independent Professional Baseball team), and worked the Boston Marathon for years. He comes to us after working with three Division I universities (Brown, University of Texas Rio Grande Valley, and most recently Florida A&M University). Thomas joined JOI in August of 2018 and serves as the Head Athletic Trainer at The Bolles School.

While in school at UNF, Greg worked with the UNF Men’s Basketball team, held an internship with the Jacksonville Jaguars during the 2010 Training Camp and Preseason, assisted the Jacksonville University Football Team as a student athletic trainer, and finished his clinical duties at Bishop Kenny High School. After graduating, he attended the University of Arkansas as a Graduate Assistant for the Men’s Tennis team and Spirit Squad. Upon leaving Arkansas, Greg accepted a job in Anchorage, AK to work with the local high school athletes for a year prior to working for JOI.

Drew has been with JOI Rehabilitation staff more than six years. He joined the rehabilitation team in 2004, and in 2009 became Site Coordinator of JOI’s largest rehabilitation facility, San Marco. Prior to working with JOI, he completed athletic training internships with the NFL’s Jacksonville Jaguars and the USAT National Triathlon Training Center. He has also served as Athletic Trainer for the Jacksonville Barracudas hockey team. As an athletic trainer and avid triathlete, he enjoys working with elite and amateur athletes who have a special interest in endurance sports, including running, cycling and multisport.

He serves as a clinical liaison between JOI Rehab sites and JOI physicians to sustain consistent and exceptional patient care and to ensure that current staff is given educational opportunities to develop and enhance clinical skills.

Adam is originally from the Chicagoland Area and moved to Jacksonville shortly after graduate school.  While in PT school Adam fulfilled multiple clinical rotations all over the country, with a special focus on orthopedics and sports medicine. During his final year at the University of Dayton, Adam was able to complete an Independent Study at DePaul University in Chicago working with Division I college athletes. Adam started working at JOI in June 2018 and currently splits his time between the San Marco and Nassau clinics. In his off time Adam enjoys exercising, playing soccer and spending time outside with friends and family.

He earned his B.S. in Biology from University of North Florida in Jacksonville, FL in 2007 and then his Masters of Traditional Chinese Medicine from Five Branches University in Santa Cruz, CA in 2012 where he began practicing as a California Board certified licensed acupuncturist in 2013. He maintained a general private practice in Santa Cruz where he focused on Sports Injury and Pain Management whiling earning a certification from the American Academy of Physical Oriental Medicine in 2012 to further deepen his knowledge of sports medicine and orthopedics.

Matt currently maintains a general private practice in Jacksonville, FL and is an Acupuncture Physician at Jacksonville Orthopedic Institute Spine Center. He continues to deepen his knowledge of integrative medicine by working on completion of his Doctorate of Acupuncture and Oriental Medicine from Five Branches University in San Jose, CA and completing his certification in Functional Medicine from The Institute for Functional Medicine. Matt enjoys working to help patients increase their function and comfort levels by focusing on integrative approaches in medicine.

Lyndsey has worked in multiple physical therapy offices administratively. In 2010, Lyndsey joined the military and became an EMT and military medic. After serving time in the military, she worked as a medical assistant, and eventually decided to learn the skill of massage. Lyndsey attended Alpha School of Massage and specializes in Neuromuscular Massage. Lyndsey believes that massage is a special gift because it is equally as good for you as it feels. Lyndsey works at both our San Marco and Fleming Island facilities.

While at the University of Hawaii at Manoa, Matt completed his research thesis on concussion assessment of children ages 5 to 12 years old. After graduating from the University of North Florida, Justin went on to advance his educational career at the University of Cincinnati where he graduated with Masters of Health Care Administration. At JOI, Justin is heavily involved in promoting JOI via many avenues.

Matt has been a Certified Athletic Trainer since 1997, and especially enjoys working with post-operative knees and shoulders. He was the Sports Medicine Director (2002-08) the JOI Rehabilitation – San Marco office and Head Athletic Trainer for Bishop Kenny High School from 1999 to 2005. He specializes in throwing and running biomechanics, foot orthotics fabrication, biomechanical analysis (throwing, running), and soft tissue mobilization using the Graston Technique (Level II trained). Currently, Matt is the Site Coordinator of JOI Rehabilitation – San Marco office.

Laran Phelps – Lead Massage Therapist

Laran Phelps is a native of Amelia Island and Jacksonville and is a graduate of Heritage Institute in Jacksonville, FL (2003). With over a decade of experience in the field working as an LMT and also as an Instructor, Laran brings deep understanding and knowledge of anatomy, physiology and kinesiology into each session. Her passion as an LMT is in Neuromuscular Therapy, Trigger Point Therapy, Medical Massage, Sports Medicine, Physical Therapy & Rehabilitation.

She is a Member of the Florida State Massage Therapy Association (FSMTA) as well as the American Massage Therapy Association (AMTA).  Laran is currently working towards obtaining Board Certification from the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB), which is the highest elected credential in the field of Massage Therapy.

In her free time, Laran enjoys spending time with her daughters at the beach, swimming, camping, geocaching, or finding an outdoor adventure.

Alex Witter oversees the medical care for the Jacksonville Armada FC first team as an employee of Jacksonville Orthopaedic Institute Rehabilitation. He is responsible for all sports medicine department operations, coordination with physicians and other transdisciplinary team members, as well as day-to-day care.

Previous to hire at Jacksonville Armada FC, Witter spent 2017-2018 as a Resident Athletic Trainer at Florida Gulf Coast University for the men’s soccer and women’s tennis teams. In this position, Witter authorized insurance, claim filing and processing, supervised students in the Athletic Training Education Program and Pre-Athletic Training Students. The year before, Witter worked at IMG Academy where he oversaw the healthcare for the tennis athletes (~220), managed inventory for the academy, placed and manage work orders, and covered additional home events.

Witter is a BOC Nationally Certified Athletic Trainer, licensed in the State of Florida as well as being CPR/AED certified. Over the past 2 years, Witter also earned Level 1 and Level 2 HawkGrip Certifications (IASTM), Functional Movement Screen Level 1, and F-MARC FIFA Diploma in Football Medicine. He is a member of the National Athletic Trainers’ Association.

Witter is a native of Florida and working towards his Master’s degree in Kinesiology with a Dual concentration in Corrective Exercise & Orthopedic Rehabilitation and Sports Conditioning.

Shane Nall is a native of Jacksonville Beach and a local around the beaches community in his 26 years of living. As a graduate of The Florida School of Advanced Bodywork in Jacksonville, formally known as the Southeastern Schools of Neuromuscular Massage, Shane is well versed in Structural Integration Bodywork and Neuromuscular Massage Modalities. His focus is identifying the causative factors of muscular pain and how posture can create and form muscular imbalances that cause structural pain. With a deep understanding in the anatomy, physiology, and kinesiology of the body, Shane uses multiple Medical Massage modalities to treat the pain people experience throughout daily life such as; Neuromuscular Therapy, Trigger Point Therapy, Myofascial Release Technique, and Sports Massage.

Massage Therapy and Bodywork is Shane’s passion, he is a member of the American Massage Therapy Association (AMTA) and currently working towards growing as a LMT, with multiple courses and certifications lined up in the future. Shane works at both our Beaches and South Rehabilitation facilities.

In his free time, Shane enjoys traveling to different surf and winter snowboard destinations. He also scuba dives and has a passion for any fun outdoor adventure.

Jon is a Jacksonville, FL native who attended the Providence School where he also played baseball. Jon was very active in sports growing up. Upon graduating, he began his career path in the Hospitality Industry in which he worked for 13 years. He chose this industry because of his love for people and love for helping and providing a service. After obtaining his A.A. at FCCJ, he moved to Orlando, FL and attended UCF’s Rosen School of Hospitality Management. Upon completing his time at UCF he returned back to Jacksonville, FL and began managing a local restaurant. Several years after beginning his management path in the industry he decided that he wanted to still serve and help people, but in a different way. He returned to school while continuing to work in the industry and for Baptist Health as a PAS representative. Upon completing his studies he began his career with JOI as a Physical Therapist Assistant. In August of 2012, he accepted the position of Spine Centers Coordinator. In March of 2015, he accepted the position of Sports Center Coordinator. He currently treats patients at our Sports Center location full time.

Chris has worked in numerous clinical settings, with over 20 years of experience working with upper extremity rehab in outpatient orthopaedics settings. She has been with JOI since 2009.

Robert is certified with the National Academy of Sports Medicine as a Performance Enhancement Specialist (NASM-PES), and has had training in the Graston Technique.

Blake graduated with his Doctorate of Physical Therapy in 2010 with an emphasis

in manual therapy.  He has worked in outpatient orthopaedics his entire career and he

was the recipient for the 2012 Jim Harrison Award for Excellence in Patient care. He is

GRASTON Technique Certified and SFMA Certified (Selective Functional Movement Assessment).
