JOI Rehab Point Meadows is located on the Southside and Baymeadows area of Jacksonville, FL near the Ikea Store.
The JOI Rehab Point Meadows location offers the opportunity to transition from your rehabilitation program to an individual supervised training program. Our transition program will allow full access to our facility and help you feel comfortable while using gym equipment. Patients are supervised by Physical Therapists and receive guidance with exercises. This a great option to continue with your recovery to regain a normal and healthy lifestyle.

The Point Meadows location is located off Gate Parkway by the new IKEA. Our rehab clinic offers Physical Therapy, Sports Rehab, DME or Bracing, Graston Therapy, KT Taping and Return to Sports Services. We work closely with your JOI Physician, but we also work with any physician who prescribes one of our services. Our goal is to return you to your prior level of function with good customer service.

Justin Carmel, PT, DPT
Bachelor of Arts in Marketing (University of Cincinnati, 2007)
Doctorate of Physical Therapy (University of St. Augustine for Health Sciences, 2015)
Justin Carmel is a graduate from the University of St. Augustine for Health Sciences with a specific focus in manual therapy. During his clinical experiences, he treated a wide range of sports and orthopaedic injuries and had the opportunity to work directly with various collegiate and professional athletes.
Prior to becoming a Physical Therapist, Justin graduated from the University of Cincinnati, with a Bachelors of Business Administration. He worked as a Sales/Marketing Manager for an International Manufacturing Supplier. Justin has a passion for therapeutic exercise program design/implementation, functional movement analysis, and sports injury prevention.
He is currently working towards obtaining his Manual Therapy Certification from the University of St. Augustine for Health Sciences. He plans to further his knowledge of functional movement analysis/sports injury prevention by pursuing a Functional Movement Systems Certification. Justin’s hobbies include cabrewing, whitewater rafting, kayaking, fantasy football, zumba, and powerlifting.

Fadel Taazieh Site Coordinator PT
Bachelor of Science in Athletic Training (University of North Florida, 2012)
Doctorate of Physical Therapy (University of St. Augustine for Health Sciences, 2014)
Fadel Taazieh is a graduate of University of North Florida in 2012 with BS in Athletic Training. He then went to the University of St Augustine to get a Doctor of Physical Therapy in 2014. Fadel’s interests are sports-related injuries and are pursuing further certification with manual therapy.
Fadel practiced Taekwondo for 14 years to receive a 3rd black belt. He has an extensive background in martial arts. Fadel also participates in extreme martial arts for several years. He has a great understanding of acrobatic stunts. Fadel’s other interests include watching football, music, and fitness. He welcomes new patients at our Point Meadows Clinic.
Lisa Wilson, Intake Coordinator
Lisa Wilson joined the Jacksonville Orthopaedic Institute Rehab team in 2018. She has worked in the Medical Office field for over 8 years in Customer Service. She enjoys Yoga & Meditation, Stamp Collecting, Camping and Archery. Lisa currently works at the JOI Rehab Point Meadows Physical Therapy Clinic.
